PARENTS/COMMUNITY MEMBERS ON-LINE SURVEY

West Middle School needs your input!  Please take the on-line survey to help West Middle School continue to improve academic success.

Each school in the MSD of Martinsville is going through the process to be accredited by AdvancEd.  Part of this accreditation process is to have staff, parents, and community members participate in the KEYS survey.  KEYS is an on-line tool to help schools and school districts build the internal capacity to begin this continuous improvement journey.  This on-line survey will be used as an assessment tool that yields scores on 42 indicators of school quality that correlates with high student achievement.

·          KEYS surveys are completed anonymously and results are strictly confidential.

·          Schools only know how many participants took the survey, not who took the survey.

Please complete the survey by Monday, November 16, 2009.  You only need to take the parent survey one time if you have multiple students enrolled at West Middle School.

Steps to take the survey:

1.        Open your Internet Browser.

9.        If you need to exit while taking the survey, then click on “Save and Exit.”   Write down your password for when you decide to resume the survey.  When you go in again, you will need to follow steps 1-3 above.  Then go to “returning user,” type in your password, and click on “resume.”  If you do not remember your password you will have to start again.

If you do not have access to a computer, please feel free to come to West Middle School at your convenience during school hours and sign in at the office.  You may also call and make arrangements to complete the survey after school hours.  We will have a computer available for you to complete the on-line survey.  If you have any questions, please contact Suzie Lipps, Melody Meyer, or Cindi Valentine at 342-6628.
